I hereby authorize the University of Utah College of Fine Arts (“CFA”), its agents, successors and assigns to use photographs, and/or voice recordings, and/or voice and picture recordings and authorize the CFA to use and publish or broadcast the same. I understand that such photographs and/or recordings may be used with my name, no name, a fictitious name; and may be intentionally or unintentionally altered. I also understand that such photographs and/or recordings may be displayed, reproduced, published, and/or broadcast through any media including, but not limited to print, broadcast, and/or digital media, and used for the purpose of publicity, advertising, trade, art or any other lawful purpose without limitation as to optical or audio effect produced and/or time of use.
I hereby waive any right that I may have to inspect and/or approve the photographs and/or recordings or the advertising that may be used in connection therewith, or approve the use to which the photograph and/or recording may be applied.
I hereby release and discharge the CFA, its agents, successors, and assigns from any and all claims arising out of or in connection with the use of such photographs and/or recordings, including, but not limited to, any claims of copyright infringement, defamation or invasion of privacy.
I acknowledge that the photographs and/or recordings may disclose my participation in a class or other educational activity and are generally protected from disclosure by the Family Educational Rights and Privacy Act (FERPA). By signing this form, I am expressly consenting to the public release of this FERPA protected photographs and/or recordings.
I hereby grant the University permission to copy and display artwork that I have created in connection with my classes at the University of Utah on social media social media sites such as Facebook, Twitter, Instagram, etc.; university publications; on the University premises, such as galleries, halls, classrooms; blogs, newsletters; or in other media or publications. I understand I will receive no compensation from the University for the use of my artwork. I also understand that I may revoke this consent at any time by providing written notice to at the College of Fine Arts, Aaron Swenson () or an administrator in your academic unit, and also to the University of Utah Office of General Counsel at . Within 30 days of receiving written notice of such revocation, the University will take steps to remove images of my artwork from its social media. I understand that my images may be shared beyond the University’s social media accounts, the University will have no obligation or power to control such distribution beyond its own social media accounts. This instrument contains the entire agreement between the parties hereto and supersedes any and all prior written/oral agreements.
